If efforts to resolve a matter through these channels are unsuccessful, an official complaint may be submitted. Complaints should be used only when necessary and not for issues already addressed through published guidelines without evidence of unfair treatment, discrimination, or procedural error.
Procedure for Filing and Tracking Complaints
- The complainant completes and submits the official Complaint Form.
- The complainant selects the appropriate complaint category and provides a clear description of the concern.
- Upon submission, the system automatically notifies the relevant office or designated person in accordance with the established workflow for the selected complaint type.
- The concerned office/person reviews the complaint and records any actions taken, comments, and decisions.
